Sphenoid bone fracture

Fractures of the sphenoid bone occur following injury to the orbit and base of the skull. Such fractures are important since they can cause loss of vision and damage to various neural and muscular tissues. Ocular injury is also commonly associated.

일단 sphenoid bone fracture의 개념과 sphenoid bone 한 번 보고 위치 잘 익혀두고 시작해보자.

 

Traumatic optic neuropathy에서 optic nerve가 4가지 segment로 아래의 사진과 같이 나누어 지는데 intracanalicular 부위가 solid circular bone canal이라 압박시에는 공간이 없어서 혈관 압박, 허혈, 시신경 세포손상까지 일으킬 수 있음.

 

Intracanalicular segment 주위가 sphenoidal sinus bone 있는 부위라서 거기에 fx가 생기면 신경압박이 생겨 시력저하, RAPD등이 잘 나타날 수 있음.
